They do work and they don't violate the laws of thermodynamics. They work on the principle that heat energy moves from hotter things to colder things. In the process of being alive your body produces quite a bit of waste heat and in order to remain comfortable your body needs to get rid of that heat. It does that by dumping that heat into your surroundings, the air and whatever you happen to be in physical contact with. In this case the mattress. Your friend is right that your body will dump heat into a normal mattress just like it does into one of the gel pads but they are wrong that it is a closed system. Whatever secret sauce is in the mattress pad works to increase the thermal conductivity of the topper thus increasing the rate at which heat get be pulled from your body, spread out into the pad, and then released into the atmosphere. Laying on one of these is essentially laying on top of a big heat sink. Of note though is that this will only work if ambient room temperature is below your body temperature, in that case the mattress pad will actually be warming you up TL;DR: Gel pads work by being a more efficient means of removing and releasing your body heat into the room's atmosphere than a traditional mattress.
