i need to get new bedding and all i see are duvet covers. is it just a plain comforter? can i use it like one? i see there’s inserts. is that necessary to purchase? are they difficult to put together?

A duvet cover is a cover for a duvet insert. A duvet insert is essentially a "blank" comforter. It's far easier to wash a cover than a whole comforter.

Duvet cover is like a pillow case for a comforter. You get a nice down comforter in white but you want your covers to have some color? Buy a duvet cover in whatever color/pattern you want and BAM! you have the color at a fraction of the cost of a new comforter. Plus you can machine wash the cover (whereas you’d dry clean the comforter). Also, some people use a duvet cover and then no top sheet on their bed so making the bed is easier.
What the heck is a duvet?
Ok, so... Whereas a bedspread or quilt are pretty flat and solid, a duvet is a fluffy blanket stuffed with feathers or some synthetic that mimics feathers.
They come in different sizes, just like sheets, but it's common to see full/queen duvets, which means they'll fit either size bed.
They also have different levels of warmth to choose from. Where I live, they're called Tog ratings. Some duvets will be okay for year round, others might be too warm for anything but the coldest months.
They're often for sale rolled up by pillows, so if you are looking by blankets they might be easy to miss, depending on the store.
Depending on the fill, duvets can be a real pain to wash. I would just get a plain one and a cover, which you just change out and wash like you do with normal sheets.
As far as fillers, it's complicated. There are a lot of guides and you can spend a little or a fortune. Someone else might have some advice there. But I bought at least two of mine from ikea and one from some fancy store, and I like them all equally.
I just use a fitted sheet and a covered duvet. No top sheet. That's how we do it in Europe. But I know my American friends think I'm a heathen without that top sheet. 😂

Some people use duvet & comforter interchangeably. Other people consider a comforter to be something intended to be used on its own while a duvet/duvet-cover is meant to be held inside of an easily washable cover but you can call it a "comforter cover" if you want. There's no rigid distinction between the two. Putting a cover on your comforter makes it way easier to wash your bed linens because you can run a cover through a normal washing machine without major problems. More on reddit.com

Solid opinion. It's way easier for me to wash a duvet cover than an entire comforter, though. I can also change it easily depending on the season/weather.
