I find the neck hole to be too small on the 100615 sweatshirt as well. Nearly all of the crewneck or hooded sweatshirts seem to be that way. You could try one of the older 1/4 zip styles, or the 100617 1/4 zip . The neck opening will be slightly smaller in a Medium. Shrinkage should not be more than 2-4% over the life of the garment--if you follow the care instructions. If you wash cold and hang dry, shrinkage will be less than 2%. Shrinkage is always more noticeable in length than width. Hoodies are sized by chest, not height or weight. You state your chest size is 42". Go Medium, however try on be Quarter zip, as I've found that the Pullover will shrink at the neckline after washing it a lot, also over time all hoodies do shrink a little bit, and you as a human are more likely to increase in size, compared to decreasing.
