Shouldnt a managed Wordpress use the latest versions of PHP, MySQL etc?
As for the PHP and MySQL version: Well, not necessarily and it depends. The version is but that, a version. What's more important is whether or not the PHP and MySQL used are secure, stable and at the version required for WP. Depending on the environment other factors come into play as well, such as it being supported by a supplier, a planned release cycle and things like that. So, if the version of PHP used is, say, 8.1-x and it is a stable version without known security issues (or with backported security fixes), and WP requires 8.0 or higher then there's no real issue here. Upgrading to the latest version shouldn't be a goal in and of itself. More on reddit.com

Should I update from PHP 7.4 ?
PHP versions below 8.1 are no longer being supported ( https://php.watch/versions ) i.e. if a bug or vulnerability is discovered in 7.4 or 8, it ain't getting fixed. There's not really any reason to NOT upgrade. Recent versions offer performance improvements. You should always be running up to date WP, theme and plugins anyway - if you're running a plugin or theme that requires php 7.4, it's just a matter of time until you're hacked, because the theme/plugin isn't getting patched.
