I don't have experience of this on Lastpass, but I know that 1Password users on Mac need to download the desktop app to install the Safari extension. Something changed at Apple which meant this was the only way to get it to work. Have you tried installing the desktop app? Answer from hawkerzero on reddit.com

Hi, I'm really struggling with this, I've just got a new macbook and unable to download the lastpass extension, it just sends me to the desktop and downloads the desktop app. I'm completely lost, the documentation says to download the safari extension, but I simply can't seem to get to it. Any help would be much appreciated as LastPass support guy couldn't seem to help either

August 25, 2025 - My issues was that I attempted to install Lastpass via the Mac App Store (it has a 2.6/5 rating for a reason). After installing it, it did not prompt me to install the Safari extension. Uninstalling it and installing the direct download from LastPass worked for me.
