Hi, quick question I'm sending a 1kg parcel from the UK to the USA (its a handmade gift).
I've filled out the UPS forms online, and created a "commercial invoice" - apparently still required for a handmade gift, albeit, of no monetary value.

I have the invoice in a PDF format to print. At the bottom of each invoice it has an empty box saying declaration statement (see below). I understand I need to sign and date all 3 invoices. Do I need to write a statement in that box, or is it just for signing and dating?

Do I attach all 3 invoices in a pouch on the outside of the parcel? Shall I print it twice, to put an additional one inside the parcel, for good measure.

I am a Canadian traveling to the US for a conference. My company is shipping a banner for our table, and I need to fill out a commercial invoice.
Should I put our company address under both the "exporter" & "consignee" sections since we are shipping it to ourselves? Or, should I put the address that the conference is at?
I have two boxes stuck in Louisville worth about $6,000 in clothing purchased in Tokyo. I'm not sure what forms the shipper (who is a clothing business) filled or did not fill out, but when I spoke to someone at UPS last night, he said the Commercial Invoice needed to be resubmitted. He emailed it to me and I see that it requires the Tax ID of me, the recipient. But I don't have one, I'm not a business. (Separate problem, there was also a bag of granola in the box and I don't believe that was declared, and the shipper has not been sent the form to declare that.)
Any idea about what I should do? I can't get through to UPS now and they won't respond to this question on email.
