How to replace Google Lens with the old Google Reverse Image Search ?
Download & install this extension - Search by Image: https://chrome.google.com/webstore/detail/search-by-image/cnojnbdhbhnkbcieeekonklommdnndci?hl=en Then go to extension settings and uncheck everything (on the left) except Google Images. This makes it one-click from the context menu to search by image. There is also the chrome flag chrome://flags/#enable-lens-standalone but google may remove it in the future, whereas the extension way should keep working. More on reddit.com

Not entirely. It depends on the type of image, how clear it is, and how much unique detail there is. The more unique the image, the more chance Google will find the exact one.
