How are Ozempic and Wegovy the same drug (semaglutide) but treat two different things?
You're getting a lot of mixed answers. It is mostly a marketing decision, but it also has logical reasoning with regulatory and insurance coverage in mind. Both are semaglutide, but the branding helps to ease communication between patient and doctor and insurance (and limit errors) since different drug doses are approved for the two indications (diabetes vs weight loss). They also (currently) have different pens-- Ozempic is a multi-use, Wegovy are single-use-- so that also helps to reduce potential errors there. More on reddit.com

They’re identical. Wegovy just happens to titrate up to a higher therapeutic dose (Ozempic maxes out at 1mg) and is covered differently by insurance since it’s explicity prescribed for weight-loss rather than type 2 diabetes.
More on reddit.comRybelsus Mexico vs. Wegovy USA
One-year Rybelsus user here. You may already know this from the package insert, but Rybelsus needs to be taken on an empty stomach, as early as possible in the morning, with max of 4 oz of water. The pill has a special coating to help it adhere to the lining of the stomach wall. The semaglutide is then absorbed through the walls of the stomach, not the intestine like most meds. You cannot take multiple pills at the same time nor with other meds - this ensures you get the benefit of the drug in the same way Wegovy works. My sister is on Wegovy and I am on Rybelsus and we have had virtually the same success. She’s lost 75lb and I have lost 65. I am nearly to goal. I personally believe the 14mg dose is similar to 1mg Wegovy (based on the Ozempic dosing schedule of .25, .5, and 1.0 which might be the same as Rybelsus 3mg, 7mg, 14mg). So you wouldn’t get the same amount as the 1.7 or 2.4 Wegovy, but the drug can be really effective at the lower doses. That is an awesome find in Mexico and I don’t think you wasted your money at all! Good luck!! More on reddit.com

Is it just marketing or semantics? They are both administered weekly with similar doses yet insurance companies will die on the hill that you can't have ozempic if you're not type 2 diabetic. Is there something else in the drug that makes it work different? Or is it just ozempic has bigger doses? If there isn't a functional difference besides dose size it's like saying you can only take Advil if you have migraine but can take ibuprofen for anything else.

It is neither. It’s drug regulation and formulation. It comes down to the differences between active ingredients and fully-formulated medicines. Saline can be used as an intravenous fluid or as a nasal rinse, but you wouldn’t use the same solution for both. You wouldn’t make them the same and you might subject them to different standards of testing, because they are for different end goals. Semaglutide is a peptide. Ozempic for diabetes and Wegovy for weight management are subcutaneous injections of semaglutide tested and approved for different uses. You shouldn’t take Ozempic if you don’t have type 2 diabetes because that’s what it’s approved for. Ozempic being prescribed off-label for much-wanted weight loss is why many people have faced a shortage of their much-needed diabetes drug. That situation is one of the reasons we might formulate the same active ingredient into two different medicines for two different patient populations. I believe the dosing is opposite of your proposition, though. Ozempic is supposed to help achieve glycemic control on people with diabetes and has weight loss as a side effect. Wegovy is a higher dose of semaglutide because it’s not being used in people with blood sugar problems it’s targeting Ozempic’s side effect as its main effect.
