Ignore me if this doesn’t work, but maybe you can think about this in the context of KonMari clothing storage. What she says for clothes is that instead of using a drawer to stack all your t-shirts flat, like you’d see in a clothing store, you should fold them so you can see each vertically, as if you were looking down at the bindings of books. In your case, you are basically trying to make two shallow drawers out of your one deep drawer - a flat layer on the bottom and then a second tier of things lying horizontally. This is tough because something is always on the bottom. So instead, can you get most things vertical so no category is “under” any other category? Forks and spoons upright in a caddy. Towels stacked all the way to the top (not under the second tier). Gadgets more vertical if at all possible so you’re seeing them by their handles. So like the title says, I'm working with a drawer that's 10.25" wide inside and am trying to find a good cutlery tray for it or creative solution. I have a basic one currently but it only has 4 vertical slots and right now my dinner and dessert forks have to double up and squish into a slot. Every holder I can find online that's 5 wide or more seems to be 12-13" wide at a minimum which won't work. However I put the utensils side by side and can easily fit 5 rows wide in the space even with gaps for the walls between each utensil. Maybe I just have weird drawers, but does anyone have any ideas? I'm half considering just giving in and buying a 3d printer to make a custom one. However I'd much prefer a premade solution if possible, both for cost and simplicity. Thanks for any help you can offer.

Two of these organizers side by side would fit in your drawer and give you 6 slots. This is the setup I have in my kitchen and it's working well for us. The two small slots at the top of the organizer are pretty much useless, but the big slots are great.

There’s quite a few options on Amazon when you search “narrow utensil organizer”. I had no such luck on Amazon when we bought our house 4 years ago. Looks like they now sell the one I found on some obscure website. You can find it under “Yamakazi Home Expandable Drawer Organizer”.
