Interviewing for a Creative Director role and have been told to have a case study ready to show. I have an outdated (2017) portfolio site but it's not applicable since that positions me as a designer. What's the difference between a portfolio and case study? Assuming it's the context you provide as to what role you played, the ask, the deliverables, strategy-etc. But is there more to the difference?

It's not the difference between a portfolio and a case study – it's whether or not you include a case study. Or really, how much depth and detail you provide. Some portfolios only have images, or maybe a client name or project title. That might work for fine artists, but not for designers. Other portfolios might have a sentence that says something like, "Logo and branding for a car wash." which is barely any better than the above. To be considered a case study, I would say it needs to include the things you mention: your role, what the client needed (try to work in a short description of the client and industry), how you presented a strategic solution, client feedback, challenges, possibly technology used (website platform, app, print vendor, etc.), and then the outcomes including, if possible, real tangible results like X number of new customers, $X additional revenue, X signups, etc. Some portfolios present every project that way, so they're long reads (most people won't do more than skim a couple) while others will present all the projects in a more brief style with one or more set up, often in a separate area, as case studies. That's what I would imagine the creative director you spoke with means. You can do just a few, but I would put in the effort to write all of your project descriptions that way. You're likely to get better results.
