This has been driving me crazy for years now. I've had the same iCloud account since my iPhone 5 I think and I've been wanting to update it and couldn't. I'm not sure if this is only in iOS18 but im running the latest dev beta on my iPhone/iPad.
I was able to go into my iCloud Mail settings, then select my alias, and select set Primary. It failed the first time I tried. Then after the second time it worked! iCloud mail flow was interrupted for a few minutes but then everything updated and worked.
I still see my old email in System Prefs but if I Logout/Login it will show my new primary email.

That’s just for your Apple Account, not iCloud Mail. In Mail Settings it still says Aliases can’t be made Primary Email Addresses. I am also on 18.1.

So I'll use this for online shopping accounts or some account I'm setting up at a store I've never shopped at before. I'll hit Okay. It's says it's created and you can see I've got it there. I can switch between them here and I can see this email address here. Now notice I also have the ability to disable the alias.
