The first depiction of a sandworm with 3 radially-symmetrical mouth flaps I can find is an illustration by John Schoenherr for the first part of the serialized "The Prophet of Dune" in Analog, January 1965.
The same idea was used as the cover art for the third part in March of the same year:
Schoenherr refined this concept and the version of a sandworm shown on the 1978 Dune calendar was very similar to later depictions, including a ring of many inward-pointing teeth:

The worm's behavior toward Paul
As others have said, off-screen thumper, 30 second delay where we hear nothing but see the worm staring them down. I think we need to remember the worms feel vibrations in the sand well before any human would perceive the thumpers acting. If the worm was chasing a sound, Paul and Jessica, then suddenly lost the same vibrations as they stepped up on the rocks and found it again a bit off in the sand (the thumper) then it might take a second to calibrate and go. It's a Guild Navigator, which has injested so much spice that it has turned into that. I dont believe the navigators were described in the first book, but you meet one early on in the second. More on reddit.com

The sandworm design has the same issue as depictions of the God Emperor - you get to pick between accuracy and looking good. The new sandworms seem more accurate with no beaks and the baleen-looking teeth. But they don’t look as cool as the Schoenherr depictions (which is ironic because Frank was 100% on board with Schoenherr’s version). More on reddit.com
