Just a suggestion - if you already have some contacts in Google contacts - export them (in CSV format, for example) and that would be your template. If you don't have any contacts in Google Contacts - create one with all required fields filled in and then export it. More on reddit.com

Hi There, I have a Google Sheet with first and last name, address, phone, and email information and am trying to create the individual persons contact in Google Contacts. I've converted the Google Sheet into a .csv file and have uploaded it into my Google Contacts. It does create a contact for each person and inputs the first and last name correctly. However it just throws all of the other information (address, email, phone number) into the "Notes" section.
Is there a way to get the email, phone, and address info into the correct fields or do I need to manually add the detail for each contact? Thx!

I would imagine the issue is your headers in the csv matching Google's logic. Here's what Google Contacts Exports (excuse the formatting).
