What does this mean? Slightly high T4 and high TSH.
You had over range or near TSH all of last year. High TSH indicates some type of thyroid disease. What time of day were all of these blood draws? High TSH with high in range FT4 could be due to Hashimoto's and/or, possibly low FT3. T4 is converted into T3. Hashimoto's can cause problems with this conversion. You'll have to retest, since TSH, FT4, and FT3 all need to be tested as a group. Blood draw needs to be first thing in the morning. Both thyroid antibodies (TPO/ab and TG/ab) need to be tested to rule out Hashimoto's. It's very important to know if you have an autoimmune disease and if your hypothyroidism is caused by autoimmune Hashimoto's. Which type of thyroid replacement hormones and doses did you try when you experienced intolerance? What sort of symptoms did you experience, and what were your TSH and FT4 when you tried thyroid replacement hormone? More on reddit.com

High TSH and high T4?
I’d say, “hey, I respect you want to wait, but I would like to try to increase my dose. I can’t think straight, fatigued, irritable, brain fog. I’d like to try 75mcg to see if I can feel better, and I’d like to retest in 6 weeks to see the results.” I’ve found dictating works better than asking. It’s not like you’re asking for a narcotic. And honestly, those ranges - could be anyone’s ranges. My husband who is a bean pole might feel normal on the higher range. My normal is a TSH of a 1.0. If they don’t listen to you, just find someone new that will listen. More on reddit.com

A TSH level lower than normal indicates there is usually more than enough thyroid hormone in the body and may indicate hyperthyroidism. When hyperthyroidism develops, free thyroxine (T4) and free triiodothyronine (T3) levels rise above normal.

Hypothyroidism in Children | Children's Hospital of Philadelphia
Hypothyroidism is diagnosed when the TSH levels are above normal and T4 levels are below normal. A less severe or earlier form of hypothyroidism is reflected by an elevated TSH and a low-normal T4, a condition called "compensated" or "subclinical" hypothyroidism.

How to interpret thyroid function tests - PMC
In the presence of suspected or confirmed central HPT dysfunction, TSH should not be considered to be a reliable marker of thyroid status and FT4 (±FT3) levels must be used to guide management. Common pitfalls in this setting include misinterpreting a low or undetectable TSH in a patient taking T4 as signifying over-replacement, or assuming that a normal TSH level equates with euthyroidism.5

Primary hypothyroidism occurs when the thyroid gland does not make sufficient amounts of thyroid hormone. It is usually characterized by high levels of thyroid-stimulating hormone (TSH) and low levels of the thyroid hormones triiodothryronine (T3) and tetraiodothryonine (T4).

TSH tells your thyroid how much hormone to make. Normally, if your T4 levels are too low, your pituitary makes more TSH to make your thyroid work harder. If your T4 levels are too high, your pituitary stops making TSH.

November 29, 2023 -

I’m a 29 year old female and with some blood tests just discovered I have low TSH and other high thyroid tests. I will attach below. What does this mean? Everything I google says, Graves’ disease, hashimotos or overactive thyroid. My husband and I are trying to conceive. We recently suffered a miscarriage back in September. I wonder if this had anything to do with it.

Some studies suggest that low TSH with high T3 and normal T4 may indicate biochemical hyperthyroidism, while other studies suggest it could be related to the slow response of the brain-thyroid axis or other underlying conditions.
