Euthyroid Hyperthyroxinemia: Practice Essentials, Pathophysiology, Etiology
Euthyroid hyperthyroxinemia is defined as a condition in which the serum total thyroxine (T4) and triiodothyronine (T3) concentrations are increased, but the thyroid-stimulating hormone (TSH) concentration is normal and there are no clinical signs or symptoms of thyroid dysfunction.

Subclinical Hypothyroidism: Deciding When to Treat | AAFP
February 15, 1998 - Its very sensitivity, however, may create a dilemma, since some patients are found to have elevated serum TSH levels, suggesting hypothyroidism, but have normal levels of thyroid hormone, whether measured as free thyroxine (T4) or free T4 index.

High TSH, but normal T3 and T4
Absolutely we can have hypo symptoms when TSH is High, but T4 and T3 are normal! The time to begin treating hypo is when TSH is High. If we wait until T4 and T3 are Low, then we may develop very unpleasant symptoms and possibly even other health conditions. I briefly went clinically hypo (Low T4 and Low T3) and had awful digestive symptoms and heart burn. I had never had heart burn before! I also felt depressed and unmotivated. I was surprised by how long it took my mental health to get back to normal. When you are being diagnosed with hypo, they often want to re-check your thyroid hormone levels to confirm that TSH is staying Abnormally High. Once they confirm this, then they are likely to prescribe thyroid medication. They know that the High TSH wasn’t just a fluke, that it’s here to stay, and that you can benefit from medication. If you are pregnant or trying to conceive, TSH needs to be below 2.5. If you are pregnant then Dr should likely prescribe thyroid medication immediately instead of waiting to re-test. The fetus uses our thyroid hormone to grow and develop normally during 1st trimester. Sometime after first trimester you still need normal range TSH to be as healthy as you can be, but baby’s thyroid gland will likely be producing thyroid hormone for baby. More on reddit.com

High TSH normal T3/T4
some people told me it could be a fluctuation and once I start the external hormone I cannot stop it ever. This is wrong. The thyroid axis is not like the adrenal (cortisol) or androgen (testosterone) axis. In these, you suppress your own hormone production and it may never come back. The thyroid axis always comes back when you stop medication (minus autoimmune damage that may have occurred in the meantime). More on reddit.com

Normal T4 high TSH
One of the many reasons that TSH trumps the other tests is that it’s a more universal test: meaning, a high TSH does mean your body needs more thyroid hormone, even if FT4 is within range. And if it’s suppressed, even if FT3 and 4 are still within range, that is your body saying there’s too much thyroid hormone circulating. FT3 and FT4 are a lot more particular from person to person. A specific measurement might be within range but actually be too low for your body. You should trust your doctor on this. A TSH of 10 is not a “maybe”. It’s not normal. More on reddit.com

June 8, 2020 - Estrogens, such as in birth control pills, or in pregnancy, cause high levels of total T4 and T3. This is because estrogens increase the level of the binding proteins. In these situations, it is better to ask both for TSH and free T4 for thyroid evaluation, which will typically be in the normal range.

A normal Total T4 level in adults ranges from 5.0 to 12.0μg/dL. A normal Total T3 level in adults ranges from 80-220 ng/dL. Free T3 assays are often unreliable and not routinely used to assess thyroid function.

A high TSH level indicates that the body does not have enough thyroid hormone. A TSH level lower than normal indicates there is usually more than enough thyroid hormone in the body and may indicate hyperthyroidism.

Doctors may refer to these measurements as "free" T3 and T4 (FT3 and FT4). What's considered normal varies depending on things such as your age and the exact testing technique used by the laboratory. Find out more about thyroid function tests on the Lab Tests Online UK website · If your thyroid hormone levels are high, you may be referred to a specialist for further tests to find out what's causing it.

A TSH measurement alone is not accurate in diagnosing central hypothyroidism, since TSH may be normal or low-normal while FT4 is typically below the normal range. Measuring FT4 without TSH may lead to an incorrect diagnosis of a thyroid disease.

March 15, 2024 - It is well recognized that in some thyroxine-treated patients with goitrous or dysgenetic CH, but without any additional defect in genes mediating TH action, circulating TSH remains raised despite FT4 concentrations being within the normal range, such that a higher dosage of T4 (with resultant elevated FT4 concentrations) is required to normalize TSH (67, 68).

July 21, 2025 - This is a lack of T4 production due to an issue with your thyroid that’s causing your pituitary gland to release excess TSH to try to stimulate your thyroid into making more T4. Having high TSH and high T4 levels may indicate that you have an issue with your pituitary gland that’s causing it to release too much TSH, and thus triggering your thyroid to make excess T4.

Subclinical Hyperthyroidism: When to Consider Treatment | AAFP
June 1, 2017 - A clinical history can distinguish subclinical hyperthyroidism from other causes of low TSH not related to thyroid overactivity, such as the use of certain drugs (e.g., dopamine, glucocorticoids); nonthyroidal illness (sick euthyroid syndrome); pituitary (TSH) and hypothalamic (thyrotropin-releasing hormone) deficiencies; and psychiatric disease, especially affective disorders. In general, free T4 and T3 levels tend to be lower in persons with these conditions, whereas persons with subclinical hyperthyroidism may have free T4 and T3 levels in the mid to high reference range.

Problems with the assessment of thyroid function and levothyroxine replacement levels in pituitary disease - Pituitary Foundation
July 4, 2023 - Thyroid blood tests (“thyroid function tests” or “TFTs”) commonly measured by the laboratory include free T4 and TSH. The range of free T4 seen in normal healthy people is very wide (broadly from 10 to 25 pmol/L) but each individual normally runs at a fairly constant level somewhere within this range. This in turn means that a level of free T4 near the bottom of normal might be too low for some people (who usually live higher in the normal range) and equally a level near the top of normal might be too high for someone who normally lives near the bottom of normal.
