If you’re on thyroid hormone during pregnancy (levothyroxine) — how often was your dose increased during pregnancy?

Background: working with an endocrinologist for monitoring (labs every 2 weeks). 17 weeks pregnant, and my levothyroxine dose has increased every month. Went from 12.5mcg to now 88mcg…. Seems like my TSH levels will be stable (2-3) for about a month before creeping high (3-5) again, and my dose having to be increased.

I have Hashimotos, but did not need to medicate pre-pregnancy because my TSH was usually 4-6 but with no symptoms. About 5 months before getting pregnant went on 12.5-25mcg levothyroxine and that made me HYPERthyroid (TSH <0.45!).

These constant increases have me kind of confused, and trying to understand if I should be pushing my endocrinologist for more answers (vs her just increasing my dose every 2-4 weeks).

I have extreme hypothyroidism, and have my entire life. I would have to go to my primary every 3 months to get my TSH & T4 checked. When I got pregnant, I was already on 125mcg and checked my levels with my OB at my first prenatal appt which was 9 weeks and they were 2.4. My OB wants me between 0.4 and 2.5 during pregnancy, so I stayed on that same 125mcg script I had pre pregnancy for awhile. Having thyroid issues my whole life, I can tell when it’s not where it should be and at 17 weeks I had her check it again. I was right, and it had jumped up to 3.3 so she increased my levothyroxine by 25% and I now take an 25mcg pill in addition to my 125mcg every morning totaling 150mcg. I am now currently 25 weeks and just got my levels checked again at my appointment last week, and I was sitting at a 1.7, so am still taking 150mcg daily! For me, it’s more so she tests me whenever I request it but again I have been dealing with this and having symptoms of it my entire life so I know my body enough, but it’s not something they have ever checked without me asking. Pregnancy hormones can really mess with your thyroid and even someone with no prior thyroid issues could end up needing to supplement during pregnancy just due to the changes in our body! As long as you’re being tested often and increased as needed I wouldn’t worry too much.
