So this is a little complicated. Generally there are two ways to write dates, the short way and the long way. The short way would look like this - 12/04/2018. The long way looks like this - 12th April 2018. Which one you use if often down to personal choice but the short way is often used when filling out forms. When read aloud both are the same, you say "the twelfth of April twenty eighteen". Notice how you don't write "the" and "of", you just use them when saying the date out loud. Some people may say the year as "two thousand and eighteen" but "twenty eighteen" is more common because it is shorter.
