What do your University's regulations say? Some allow the use of paraphrasers and things like gramarly, others don't. Fundamentally you are using a tool to reword your work, so it's no longer your words.
Does this count as plagiarism if that means it is technically my own work?
Potentially, or more possibly as "external assistance" depending on circumstances and your uni's regulations. Really it isn't your work - you have written it and asked "someone else" to rephrase it for you.
Any suggestions on what to do?
Talk to your personal tutor and be honest with the AI panel/meeting. The more open you are, the more likely a less severe outcome is.
