I'm in the same predicament here. I removed the set screw, but can't get the lever to pop off. I've tried several iterations of WD-40 and vinegar/water in the set screw hole. The "two indents" are the flats that allow you to use a wrench to unscrew the cap to get at the innards. It may very well be an old fashioned washer and seat mechanism rather than a cartridge. If the shaft travels in and out even very slightly as you turn it, that's what you have and all you'll need to do is replace the rubber washer. More on reddit.com

Find someone else, about a 20 minute job and maybe $30 part. He should have had the part already in the truck when he first came as well.
