I hate it, but this is unfortunately a common practice, and not unique to UPMC. I've been paying these facility fees for well over a decade to see my gastroenterologist, but at least they're actually in Presby and not some random office. Edit: It's been a long time since I posted this but just wanted to add a comment to say that they eventually dropped the charges after I was annoying enough for long enough. They never did get me the transcripts from my call. Now, I always ask if there will be a hospital fee when I book an appointment anywhere and have successfully avoided further headaches!

Edit for tl/dr: if you’ve ever fought a dumb UPMC charge and won, how did you do it?

It’s been 222 days since I posted about a ridiculous charge UPMC threw at me for going to see a gastroenterologist for no care, just a 10 min new patient appointment where I got a prescription for heartburn meds and was told to come back on 3 months. I’m still working on this issue and think I’m just going to try filing a small claims suit. Is there anyone here that knows where to start with this, or who has had luck doing this themselves? I have the bill and an itemized statement with billing codes that says I should have a $0 balance aside from the “hospital fee” they’re trying to charge me, even though I didn’t go to a hospital and didn’t receive hospital-services. I’ve been trying since March to get a transcript from the call where they said I “may” get charged a hospital fee, since I know that on that call I was also told that I wouldn’t actually be charged the fee, since I was going to just be visiting a regular doctor, not in a hospital. No one has been able to get that for me yet. Would love any help! (Also I don’t know how to link my original post eek sorry but there’s more info there)

The PA attorney general actually has pretty good health care billing resources and can help you navigate this. You can also file a complaint with them if the provider and your insurance cannot help you resolve this issue. I love fighting ridiculous medical charges, but from my experience with this particular issue (also a UPMC GI doctor), you may not have success. I’m just warning you. I know that it’s complete bullshit, but no one will care what was said on the call, probably. The onus is on the patient to check with their insurance to confirm coverage prior to the appointment regardless of what the doctor’s office tells you. No one does this of course, because it’s a dumb thing to have to do. You probably signed something in the mountain of papers they handed you saying you’d be subject to the fee as it was a hospital-based clinic (does not need to be in an actual hospital to be a hospital-based clinic because our system is BROKEN) and they’ll try and make you pay because of that. Also, your insurance should absolutely be helping you more than they are. What does your Explanation of Benefits (EOB) from your insurance say? Does the total from the EOB match what UPMC is billing you?
