Running into the same issue. I’m at the sign in screen. QR code and Amazon.com/mytv won’t work. I can scan the QR code and enter the code, but neither will refresh the app on my end. It’s been a few days now. I’ve restarted the Apple TV, uninstalled/reinstalled, tried multiple times. Answer from eightdotthree on reddit.com

Ok so I ran into this same issue on the bedroom apple tv. I'll tell you what's wrong. Amazon has recently enforced a device limit which means only 5 devices (out of which 2 tvs can have prime video active on one amazon account). If you are in the same boat as me, you might want to open the prime video app on your phone and navigate to the devices section where you can remove old devices. In my case, out of ignorance I hit the monthly limit for removing devices which means I'm stuck without prime on the bedroom tv. Will have to wait until March 15th to be able to remove more devices and hopefully log in. If that fails I will have to buy a seperate plan for the in-laws so that the 2 allowed tvs are in my household.
