This link (at the bottom) may give some insight on how Google App Script can be used to get info from a QR Code, but it doesn't reference pushing that into a google form; but the short answer is no, it doesn't look like it's possible natively through Google Forms. I'm also not sure Google Forms could easily solution your need because it's not designed to dynamically insert additional response fields. In your example you had three items, but if another ticket had 4, and a third had 7 items; you'd need to design the form to handle up to X items. There's ways of doing it "nicely" from a UI view but still not a great solution. The closest you could easily get would be to make a pre-filled form for each inventory item, and make those URLs the QR code.
