Yes, things have changed, mostly for the better. Additionally the testing methods have also changed, and a car that was "5 star" in the past, wouldn't necessarily pass the same tests today. They didn't used to do a minimal offset test. They didn't used to test headlight effectiveness. Cars still have safety cages, and crumple zones as they did 10 years ago, but manufacturers are always working on improvements. Then there's all the extra tech, like blind spot monitoring, which doesn't exactly make your car safer, but does help you as a driver to notice things better, which results in fewer accidents. I'd still rather have NO accident, then to have an accident in the safest car.

Cars models are essentially the same for 6-10 years depending on brand. Find a car thats on both the 2023 and 2020 list and you should be fine there.
