My understanding is yes they do need to be included. I ship high value occasionally and when I get these forms I just take the label and the forms to a post office. They have these little clear pouches that adhere on to the outside of your package where your customs form and label can go. I just ask the behind the counter clerk to help me put everything in properly. Phone numbers aren't necessary but good to have in case the delivery courier in Germany needs to contact the recipient More on reddit.com

If you use USPS Click-N-Ship, it will generate the customs form based on the destination country. I don't know about other postage clients but my guess is they do the same. More on reddit.com

Is one of the forms a Form 2976? Or is it a Form 2976-A? If it is the latter, then you must insert it into a Customs Declaration Envelope (Form 2976-E) which you can get from the post office. It is possible that the carrier might have one available if you do not. When you print an international Click-N-Ship label, you receive the label and the needed customs forms at the same time. If you print any Priority Mail Express International® label or Priority Mail International® labels, you will receive 2 pages of information and one page of instructions. All of the labels/customs forms will be placed in the PS Form 2976-E envelope. This envelope is required and can be ordered through the Postal Store . I bought a label that looks like this.
It also came with page 2 customs declaration, page 3 dispatch note (and page 4 senders copy).
It seems like these three are the same except the first page has the barcodes. Do I need to include pages 2 and 3 (on? in?) the package?
It's going to Germany and insured for $550.
Also, should I bother to get the recipient's phone# to put on the form?

Yes all are needed. The copy with the barcode will stay with the package. Dispatch note will be taken by the post office for their records and customs will take their copy before it leaves the US.

Hello all this ain’t meant to be a stupid question i just need some advice from people more experience I’ve never done this before. I’m trying to send a Christmas card, postcard, and a letter all in one envelope to the EU. From what I saw on stamps .com this should be considered documents and doesn’t require any customs papers. In order to get it there on time I need to use priority international and on quick ship it requires customs info and won’t let me move on. The problem is I don’t even know what to put since it’s just documents/paper? Do I take the package to usps to send without customs and have them make me a label, or do I need customs no matter if it’s a small envelope with just paper inside? What would I put for cash value/items? Thx
TLDR: Sending first international package using priority international and not sure what to put for customs since it’s just a card and letter. Didn’t think I needed it since it’s just paper and a small envelope. Tried calling post office but they won’t pick up the phone.

If you send documents international via priority, then yes, a lot of countries will require the customs form. On the lines for what's inside, you can either put documents/3, or put card/1, postcard/1, letter/1. You can't put a value of zero, but can put a value of 0.01 for each unless you want to put the actual value for the card and postcard.
