3 pages of forms for international shipment. What do I do with all of them?

I bought a label that looks like this.
It also came with page 2 customs declaration, page 3 dispatch note (and page 4 senders copy).
It seems like these three are the same except the first page has the barcodes. Do I need to include pages 2 and 3 (on? in?) the package?
It's going to Germany and insured for $550.
Also, should I bother to get the recipient's phone# to put on the form?
Hello all this ain’t meant to be a stupid question i just need some advice from people more experience I’ve never done this before. I’m trying to send a Christmas card, postcard, and a letter all in one envelope to the EU. From what I saw on stamps .com this should be considered documents and doesn’t require any customs papers. In order to get it there on time I need to use priority international and on quick ship it requires customs info and won’t let me move on. The problem is I don’t even know what to put since it’s just documents/paper? Do I take the package to usps to send without customs and have them make me a label, or do I need customs no matter if it’s a small envelope with just paper inside? What would I put for cash value/items? Thx
TLDR: Sending first international package using priority international and not sure what to put for customs since it’s just a card and letter. Didn’t think I needed it since it’s just paper and a small envelope. Tried calling post office but they won’t pick up the phone.
