So I recently took a loa because I had to get wisdom teeth surgery and my surgeon told me to take 4-5 days off so I can heal. It got approved very quickly.

The trouble I'm having now is returning back to work. Now sedgwick sent me Return back to work forms but my surgeon's office told me they didn't get any forms today. (I guess it makes sense since sedgwick faxed the forms to them yesterday so maybe it'll take a few days for them to get them??) I'm so frustrated because I should have returned to work 2 days ago and I need money, bad. I regret taking this leave. I would've been better off calling in or just going to work with my mouth full of blood. So now I've been calling back and forth between sedgwick and my surgeon's office to make sure everything is going ok (which to me it isnt). My surgeon's office seems confused on the whole process. They told me they'd write me a return to work form but I'm not sure if that's gonna be enough. I was thinking about printing off a copy of the forms sedgwick sent to me and having the staff/doctor fill it out, scan them and fax it to sedgwick so sedgwick gets them much faster. This is so frustrating. What should I do?? Help, I feel like crying :(

How do I get to go to work without this form? I'm tired of being at home
