It's almost like the app is not seeing the taskbar
I've found the answer. I've seen another forum, and it seems to be a side effect of Nvidia's Game Overlay. Disabling Nvidia's Game Overlay will fix this. Here's a link to the post I found it on: https://superuser.com/questions/1163969/windows-10-taskbar-is-not-always-on-top
New GeForce Drivers that are installed with GeForce Experience (since 2016) call it the In Game Overlay, and you can disable it by opening GeForce Experience > Settings (top right) > General and untick the In Game Overlay option.
Turning off my HUD FPS Layout in NVIDIA Settings (ALT + Z) appears to have resolved the issue for me.

If the app is opened by a shortcut, you can right click on this and go to Properties, then the Shortcut tab and change the Run drop down to Maximised.

I had to restart "Windows Explorer". I go to Task Manager and restart "Windows Explorer" process.
It is temporary, it happens again from time to time.
This issue with File Explorer is, as I noted, likely a context issue
(1) Try a Windows 11 Repair Install.
Go to the Windows Media Creation Link

Windows 11 is running, so click on the Download button (not Upgrade Button) and select Run. Then save the result to a USB Key. Navigate to the USB Key and run Setup. At the appropriate question, try Keep Everything. Allow the Repair to run. This is akin to watching paint dry.
(2) If Repair fails, then there is more serious OS damage or User Profile damage.
Best approach for this is to fully back up and reinstall Windows 11 Fresh.
