How does eBay returns work if seller doesn't accept returns?
The sellers "does not accept returns" policy only applies to buyers remorse. It doesn't mean squat when it comes to getting something that wasnt as it was described. As long as you file a return case specifically with the reason "item not as described", then you'll get your money back one way or another. The seller has 4 days to respond to the case either by selecting "issue return label" or "refund". If they do neither and just don't respond then after 4 days you can go back to the case and there should be an option to "ask ebay to step in". Once ebay has to step in, in these cases they'll generally just refund you (out of the sellers money) and tell you to keep the item since the seller refused to issue a return label. Seller will also then get a defect on their account for an unresolved "item not as described" case which will increase their seller fees. More on reddit.com

Does eBay ever refund a buyer and also allow the seller to keep their money?
Most likely the refund came from the seller. If you do an INAD case and the seller won't accept the return and Ebay has to do it, that's what almost always happens, they take the refund from the seller. If there are some extenuating circumstances, then Ebay might pay the refund, but that's rare. More on reddit.com

How do you guys live with accepting returns?
Op, returns are the best form of protection you have on eBay: If an item is returned in a different condition, you can deduct 50% of the refund if you offer 30-day returns as a top-rated seller or free returns if you're not top-rated. IMO that's the biggest ace in the hole you have as an eBay seller. eBay also automatically removes negative / neutral feedback in that situation. If your return policy is "no returns," none of those protections apply. And If a buyer has a complaint about an item, all you have to say is "so sorry to hear that, feel free to open a return." At least 50% of the time you never hear from the buyer again because they don't want to return the item. It greatly simplifies customer service. And for what's it worth: I'm full-time and had 15 returns out of 1,728 sales in the last year (1.27%). Of course I wish that number could be zero, but in the grand scheme of things, I'm OK with a platform where 98.73% of my sales go through without a hitch. More on reddit.com

I bought an item that has sold out everywhere, they are bling box dolls like the labubu but a different brand called Maymei. Seller took about a week and a half to respond and I was going to cancel once they hit their 10 business day max. Now I finally got the item which now I'm not surprised why it went through so quickly, I saw the size of the package on the delivery pictures and knew I did not get what I purchased, it's a pack of sports cards. 230$ and I just got sports cards.

Now I did a return request with pictures of the package, bag and even a delivery picture to show it's the same poly mailer and info on it. If the seller doesn't accept returns and they don't answer, what happens next?
