What does high free T4 mean?

Normal TSH but high free T4?
I think I'm in the same boat as you. I was checked 8 months ago, and they found a goitre. Ultrasound was done, and no nodules or growths. Thyroid looked nice and smooth, but clearly enlarged. Bloodwork showed a TSH of 3, with a range from 0.35 to 4. Now, 8 months later, I got blood work again, and my TSH is exactly 1. I believe I'm transitioning towards hyperthyroidism as well. I've been having these weird heart palpitations lately, and I think this is what's causing it. Anyway, stay on top of it, buddy. Keep getting blood work done and following up

One possible reason - the pituitary gland is malfunctioning, unnecessarily secreting extra TSH when not required. So your T4 levels are also high because of this high TSH asking the thyroid gland to do more, but the thyroid gland itself is functioning normally. This condition is called secondary hyperthyroidism. Are you experiencing hyperthyroid symptoms?
The second possible reason is that the ft4 result is faulty, since ft4 levels in blood are minute compared to total t4 it may sometimes be difficult to measure accurately.
