I am having the same issue. When I first installed it, it worked fine now it just keeps searching for device even though I have click play on my watch. 🤷🏻♀️🤷🏻♀️ I just use pandora now because it works.

I just figured out a hacky way to force Spotify to play from the internal speaker in the watch: Open Spotify on the watch and select music to play. Select your bluetooth headphones and allow it to start playing. Go back to the home screen, and launch the "Media Controller" app (looks like blue play button). Select Watch and you should see Spotify playing. Disconnect the bluetooth headphones. (e.g. put buds back into case). Music will stop. Press the play button in Media Controller to resume playing from the watch speaker. Note that you can't skip forwards/back after doing this, and if you go back to the Spotify app and pause the music, it will ask you to connect Bluetooth headphones again. Edit: this also works for Youtube Music, and you can go prev/next unlike with Spotify.

The Spotify watch app is a bit of a disaster. Spotify is aware and has done nothing to fix it, this is not an apple issue. You can download music but will it actually play your playlist - maybe, it may play no songs, it might play 2-3 songs, it might play 10 or 100, one never knows what is going to happen on any given day. If you want a good experience with music on your watch switch to Apple Music. You can easily transfer your playlists from Spotify to Apple Music with a variety of services and your experience will be far better and reliable.

I do not own a watch but want to get one pretty much only for spotify on the watch so I can workout without my heavy phone in my pocket.

It works fine for me. You can download songs and play offline directly to your AirPods, no iPhone needed. You can also use WiFi (maybe cellular too, idk, don’t have that model) to stream instead of downloading.

I haven’t used Apple Music, but for Spotify if you just start a playlist before taking off, you’re good to go. It’s a bit inconvenient to try and switch music playlists later, but I haven’t had any issues. My main issue is only if I forget to start Spotify before I leave home as it refuses to connect to the watch if you are already away from your phone.
